The Bow Valley PCN Interdisciplinary Health Team includes: mental health clinicians, a registered dietitian, an active living consultant and a social worker. They provide support through one-on-one consultations for adults 18+. In-person appointments at the PCN office in Canmore or Banff are available, and virtual appointments are also an option.

The PCN health team facilitates various health management classes.

All services provided by the PCN health team are free of charge. Patients can be referred by their primary care provider, allied health team, or by self-referral.

*Active Living Consultant: Prior to being admitted to any exercise program, medical clearance will need to be established.
