Bow Valley Primary Care Network COMMUNITY SOCIAL WORKER 1.0 FTE

The Bow Valley Primary Care Network (PCN) aims to improve access to primary care, increase emphasis on health promotion and disease / injury prevention and improve coordination of care through inter-professional healthcare teams.

The person who thrives in the Bow Valley PCN is excited about being part of primary care reform, working as a member of the healthcare team and embracing a patient-led, self-management philosophy.

As a valued member of the Inter-Professional Primary Care Team (IPT), the Community Social Worker (CSW) works at full scope of practice within a 2-pronged approach to provide evidence-based and equitable health care; within an individualized approach and within a social determinants of health framework.

The Community Social Worker is responsible for:

• Conducting need assessments, identifying system gaps, providing support, guidance, and facilitating access to available community resources and services.
• Assisting with system navigation and referrals to services and resources related to basic needs: housing and transportation, financial/ income support, disability support, food security, older adults and caregiver support, and other supports / community resources.
• Responding to referrals, delivering individual (1:1) consultations and facilitating small group sessions to promote optimum psychosocial functioning.
• Identifying emerging needs and health disparities and supporting the implementation of population health strategies to address these needs.
• Working collaboratively with a team of allied health professionals to support patients in meeting their needs and goals.
• Supporting the development and evaluation of the Bow Valley Primary Care Network (BVPCN) programs.

Applicants must have the following qualifications:

Education & Professional Memberships

• College Diploma in social work or an equivalent diploma
• Bachelor in Social Work preferred
• Active member in good standing with the Professional College or Health Professional Governing Body related to this role (proof of registration in good standing is required annually)

Professional Experience:

• Minimum of 2 years’ working on providing social service supports, system navigation and facilitating access to community resources and services.
• Experience working in healthcare (primary care preferred).
• Experience working with patients with diverse psychosocial concerns.
• Experience supporting equitable access and providing community resources and services to all patients including equity seeking populations (e.g., persons with disabilities, immigrants, newcomers, refugees, 2SLGBTQ+, indigenous populations, other).
• Demonstrated ability to make community connections and knowledge of existing community agencies in the Bow Valley.
• Knowledge of the social determinants of health and population health strategies.
• Experience working with primary care providers, other healthcare and community-based providers.
• Demonstrated ability and willingness to work in an unstructured and dynamic environment that requires flexibility and collaboration with other health professionals and community providers.

Other Job-Related Requirements:

• Proficient in basic computer skills (Microsoft 365 Suite, email, EMR)
• Vulnerable sectors criminal record check
• Proof of immunizations in compliance with PCN policies
• As travel may be required between clinics, a valid driver’s license and access to a reliable vehicle
• Familiarity with Occupational, Health & Safety and safe work responsibilities

This 1.0 FTE position is 38.75 hours/week.  Salary is commensurate with qualifications and experience.

Extended health benefits, RRSP matching and vacation allotment are part of the total compensation following successful completion of a 3-month probationary period.
