• disability support
• food security
• older adults and caregiver support
• other social factors and community resources

Request an appointment with a community social worker by:

Asking your primary care provider if a referral is right for you.  A referral can be made by your primary care provider, or you can contact the Bow Valley PCN at 403 675 3000.
