This free program is for individuals who have been living with chronic pain for six months or more. The goal of the program is to help you learn how to manage your pain and improve your overall function. This program does not promise pain elimination; the focus is on self-management through education. The program is 3-6 months in length.
Who will I work with in the program?
You will have access to an interdisciplinary healthcare team including: an Active Living Consultant, a Behavioural Change Specialist, a Pharmacist and a Physician. Who you work with depends on your goals.
How can this program help me?
The Chronic Pain program offers a broad range of services including one-on-one appointments and group classes. This program will support you with your goals related to active living, coping strategies, medication recommendations, stress reduction and other self-management techniques.
You can join the Chronic Pain program through either a referral from your doctor or primary care provider, or by calling the PCN directly to book in.
