PCN Nurses

Our PCN team includes both Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), specializing in primary care, chronic disease management, maternity and newborn care. Working with your family doctors and allied health professionals, our Primary Care, Chronic Disease Management and Obstetrical Nurses (LPNs and RNs) specialize in helping you reach your health goals. Whether that’s pre/post natal care or managing a condition such as diabetes or high blood pressure, our PCN nurses are available to help you navigate options to optimize your health & wellbeing.

Nurses help people of all ages stay healthy and manage chronic conditions. They promote healthy lifestyles through education on topics such as mental health, heart health, sexual health, women’s health, and much more.

Working with you
Our PCN nurses work at Bear Street Family Physicians, Canmore Associate Medical Clinic, Ridgeview Medical Centre, and Mountain Maternity & Family Medicine. If you have a family doctor at one of those clinics you can meet with our nurses there. In clinic, our nurses can provide a detailed one-on-one review of your medication history, helping you to set appropriate health goals, and many other things. With an action plan in place, our nurses will follow your progress, monitor results, communicate with your doctor and connect you with additional supports as required.

If necessary, you may receive a referral for consultations with other members of our PCN team, or to one of our programs or workshops. If you require access to other parts of the health system, our nurses can connect you to the resources you need.

Workshops
You can also meet one of our nurse team by attending one of their workshops. Our nurses also run a number of workshops each year including the MINT Memory Clinic, Happiness Basics, and Birth & Beyond.
